![]() Reviews and completes all Pre-Placement documentation.Schedules Pre-Placement Meetings if placement is accepted and invite all necessary participants.After Meet & Greet appointment, sends Provider Response Form to referring agency – Support Coordinator or DDD Community Development Unit (CDU).After placement assignment is determined, sets up Meet & Greet interviews with the individual in need of services and all appropriate parties.Works with Clinical Services, Training, and Management to coordinate training for staff.Attends training programs, conferences, and seminars relevant to program development and service delivery, as well as Agency/Department staff meetings.Assists in the review all referral packets and prepares bulleted notes along with the Senior Director of the Admission and Referral Development Department for vacancy or new development placement assignment.Encourages and utilizes the regional Executive Directors' input in program development and in the decision-making process as necessary and appropriate. ![]() Updates the Residential and Day Program vacancies and placement spreadsheets weekly.Submits all appropriate documentation within the specified timeframe to the Admission and Referral Development department and/or the funding source.Reviews and assures implementation of appropriate standards for licensing as well as contracted level of service for programs developed.Screens and selects individuals referred for services.Participates in the implementation of Residential development as necessary and appropriate.Works with the Chief Operating Officer and Senior Director of Admission and Referral Development to develop new and/or modify and continue programs, services, and funding in the Residential Programs.Works a 40 hr flexible work week, that may include evenings and weekends to attend meet and greets and new admission placement meetings.Valid unrestricted driver's license with good driving record.Proficiency in computer applications such as Microsoft Office and Database applications.Excellent organizational and communication skills – both orally and written.Bachelor's Degree preferred or High School Diploma with a minimum of four (4) years' experience in residential programs.
